The episode name comes from a missing fifth page of Maia's medical chart, which held information about the promicin inhibitor program. The episode is significant because it introduces promicin, the drug which gives the 4400 their abilities, and the promicin inhibitor]scandal, which leads to deep distrust between the 4400 and the government, especially NTAC.

4400's around the globe become ill with an unknown disease as Nina Jarvis is on medical leave. Dennis Ryland returns to take temporary command, and orders all 4400s to report to NTAC in response to this epidemic. As 4400s, including Shawn and Maia, lose their abilities as they get ill, Tom and Diana gradually make discoveries of secrets NTAC Medical has been keeping - and causing these illnesses.
Dr. Kevin Burkhoff gives his research to Max Hudson, who refuses to share classfied medical files with Burkhoff. Even though Burkhoff has been proven to be the most important scientist related to the 4400 (Tom noted "that the future built a giant tower to wake him up. I guess they think he matters.") there aren't too many people who are interested in his help. Tom and Diana convince Marco to give medical files to Burkhoff. Burkhoff discovers that a synthetic substance called promicin-inhibitor is present in the blood of every 4400. This substance inhibits promicin - which is a fifth neurotransmitter present only in the 4400 - and in the process compromises immune systems. Burkhoff realizes that if promicin can be given to the 4400 the inhibitor will be defeated. However, he can't find a 4400 who hasn't been on the inhibitor, nor can he create enough promicin to help everyone.
