The New World was the first episode of the third season. The name comes from the fact that so much has changed. This episode saw the exit of Lily Tyler, the founding of the Nova Group, and an introduction to the adult Isabelle.

Dennis Ryland tesitifies about his promicin inhibitor work. TJ Kim uses her ability to have Ryland's attorney stab him with something. Tom shoots TJ as she flees. The Nova Group, a faction of radical 4400s, announces itself. Nova declares that something has been planned for October 19. Nova kills several of Ryland's co-conspirators and Gary Navarro goes after Ryland himself. Gary Navarro is brought in from the NSA to help track down the Nova group. It turns out that he works for them. The apartment of Wesley Hauser is raided, only to find a mess and a copy of the 4400 Commission Report.
Shawn brings Richard to their living area and shows him the adult Isabelle. He believes this to be a joke, then a game. He then finds an elderly woman lying on the floor who turns out to be Lily. Lily has biologically aged to about 75 years old. Richard and Lily go to Dr. Burkhoff for answers. He ensures them that nothing like this has ever happened before. He tells them that there is a one to one ratio between what happened to Lily and what happened to Isabelle.
Isabelle realizes that she is what is causing Lily to age rapidly, and that the only way for Lily to survive is for her to die. Lily will not allow this, and Richard claims that he will not make the choice between the two of them. Lily gives Richard his birthday present - a certificate to fly an Air Force jet for 30 seconds. Lily develops several health problems, including diabetis and arthritis. Shawn helps her. Richard makes everything fly around the room while mixing Lily's pills. Isabelle jumps off the roof of the 4400 Center, and fails to harm herself.
Isabelle reads several books from an encyclopedia. Isabelle reads books about Stalin and Pol Pot in order to learn more about evil. Isabelle talks to Shawn about evil. He tells her that he slept with his brother's girlfriend, which was a betrayal of trust. Matthew Ross returns from Washington DC to help Shawn. He slaps Isabelle for trying to harm herself, and explains to her why she grew up so fast and her reason for existing. He also gives her an anti-promicin syringe which will kill her if she decides to use it. Shawn meets with Daniel Armand in an SUV in a park somewhere. They debate the work Nova has done, and he heals TJ Kim.
Matthew Ross talks to someone on the phone about Jordan Collier. Daniel Armand breaks Gary Navarro out of a security truck. Shawn betrays Daniel and arranges for him to be captured by NTAC. When TJ Kim arrives instead to collect the money, she is darted by NTAC and taken into custody. Marco shows Diana a brochure for the 4400 Center school. It is free and has great teachers. Diana does not like the idea originally, but takes Maia for a tour.
October 19 arrives, but instead of an attack, a 1,000 acre section of northern Sudan is turned from a desert into a wheat field. Isabelle decides not to use the syringe. Richard takes Lily to the cabin where the used to live, and Lily later dies in the car. Maia is shown attending her first class at The 4400 Center, and meets Tyler Downing, a fellow 4400 child.
